About the Direct Payment Holder	We are looking for a self employed Support Worker to join our team supporting a lady in her late 60’s who has previous history of stroke/TIA and newly diagnosed with vascular dementia. The ladies mobility is unsteady and she uses a frame around the house and a wheelchair for any trips out.
Hours and rate of pay hours	2.25 hours per week (weekends) ( Sat 30 mins AM & PM, Sunday 45 mins AM 30 mins PM) some flexibility to cover holiday/sickness in team. Up to £25 per hour rate to be discussed.

What you would be doing…	Supporting with washing/showering/dressing getting breakfast together in the morning, monitoring medication, simple domestic duties & shopping if time allows. Supporting with preparing food, simple domestic duties and getting ready for bed in the evening. We may require a chaperone occasionally for hospital appointments, we an arrange hospital transport for this but would need someone to attend the appointment with her, plenty of notice will be given if this is needed, and extra hours negotiated. Needs are minimal at the moment but as her condition progresses her care will need to change.
Required skills and experience	You must be registered with HMRC as self employed. 2+ years experience with dementia care and working with vulnerable adults. NVQ in health and social care would be an advantage but is not essential. Experience of moving and handling. It is essential you can drive due to remote location. We are looking for someone that is kind and compassionate about what they do, we want someone that is going to come into see the client and ask how they are, take an interest in her and her life. We have very minimal family support, so the care team need to be able to communicate effectively with the client’s family with any concerns or assistance needed. Lastly, have a good sense of humour! Once trust is gained the client loves to have a giggle and joke and it will just take the right person to bring her out of her shell

Z has a severe learning disability, genetic epilepsy (medication resistant) although daytime seizures are rare. Z loves college, in particular the bus ride there and back, has lots of friends and gets on well with his tutors. Z enjoys puzzles & books and being in the garden, his pets (a dog and a cat) are very important to him.

J lives with his family, he has lots of energy and enjoys being outside going for long walks, he loves food and enjoys signing and dancing.
Hours and rate of pay hours	89.5 hours per week in total, there is some flexibility around number of hours you can/want to work we are looking for a team of at least 4 people to share these hours. Paid at £13.90 per hour. .

What you would be doing…	Morning support – getting up, washed/showered, dressed and ready for the day as well as support preparing & eating breakfast, it is important to encourage J to complete as many tasks as he can himself to promote independence. J can follow instruction but it can take time for him to complete the instruction. Afternoon support with accessing the community and day services, community support is 2:1 to ensure J is safe so you will be working alongside colleagues.
Required skills and experience	It is essential you have experience of supporting others with Autism & Learning Disabilities. If you can drive that would be preferred, it is essential we have some drivers in our team to drive J’s mobility vehicle when accessing the community/day services. We are looking for positive role models for J to learn/improve daily living skills. Although her physical recovery has been strong, she continues to experience challenges with short-term memory, executive functioning and visual impairment.
