| About the Person You Will Support: | S is a friendly and approachable individual with complex health and care needs following a stroke in 2020 and other long-term medical conditions. As a wheelchair user, she receives support for her personal care and mobility and is looking for a reliable Personal Assistant to help coordinate and organise her appointments, activities and support arrangements. |
| Key Responsibilities: | The role focuses on helping S maintain her independence and quality of life by coordinating medical appointments, physiotherapy sessions, social activities and community access. You will liaise with healthcare professionals, family members and care providers, support S to attend appointments and activities when required, and keep basic records of arrangements and changes. There may also be occasions where personal care support is needed during activities. |
| Requirements: | The ideal candidate will be organised, proactive, reliable and a confident communicator who can work collaboratively with S, her family and professionals involved in her care. Experience in a support, coordination or Personal Assistant role is desirable, along with strong planning, communication and record-keeping skills. A driver with access to their own vehicle is preferred, and business insurance may be required depending on travel arrangements. Flexibility, a willingness to learn and a positive attitude are essential. A DBS check, references and proof of the right to work in the UK will be required.
