About me S is an adult with complex health and care needs following a stroke in 2020, alongside several long-term medical conditions. As a result of her stroke, S experiences reduced mobility, right-sided weakness and some difficulty with speech, meaning she may require additional time to communicate and be understood.

S  is currently supported by a structured care package and is dependent on carers for day-to-day personal care and mobility support, using a wheelchair and specialist equipment to support her daily living. She also has specific dietary, medication and health requirements that are managed through her care team.

Alongside her physical needs, S would benefit from more coordinated support to access medical appointments, physiotherapy, community activities and social opportunities, which are key to improving her overall wellbeing, independence and quality of life.

S lives at home and is supported closely by her family, including her two daughters, who are actively involved in her care. It is important that any Personal Assistant works collaboratively with both S and her family, while also liaising with professionals involved in her care.

S is able to express her preferences and will let you know what she does and doesn’t like. She values someone who will listen, respect her choices and tailor support to suit her individual needs, ensuring all activities and arrangements are centred around what works best for her.

The role focuses on coordination, organisation and facilitation of services and activities, ensuring S can access the support she needs in a timely and consistent way, in line with her personal preferences and care plan requirements.

 

Hours and rate of pay Rate of Pay: In line with local authority personal assistant rates- TBC

Hours: To be agreed -Likely to include a mix of weekday hours with some flexibility.

What you would be doing… • Managing and scheduling S’s appointments, including medical reviews and therapy sessions etc.
• Booking and coordinating physiotherapy and other professional support.
• Planning and organising outings, social activities and trips that promote wellbeing and independence.
• Liaising with healthcare professionals, S’s family and personal care providers.
• Coordinating support arrangements, including adjusting care visits where appropriate to fit in with planned activities or appointments.
• Supporting S to attend appointments and access the community where required.
• Keeping basic records of appointments, arrangements and any changes.
• Providing hands-on personal care during activities if required. 
The person I would like… ·        Someone who is highly organised, proactive, reliable and consistent.

·        You should be a confident communicator who can liaise effectively with professionals, family and care providers, while respecting S’s independence, preferences and decisions.

·        Experience in a support, coordination or PA role is desirable, and you should have strong planning, communication and record-keeping skills.

·        A driver with access to their own vehicle is preferred, and business insurance may be required depending on travel arrangements.

·        Willingness to learn, flexibility and a positive attitude are essential.
