| About me | PA is needed for an independent person who requires support with booking and attending various hospital appointments. To support with the understanding and implementation of treatments plans.

| Hours | Monday to Friday – 8 hours a week
Hours vary from week to week depending on the various appointments booked.

| DBS/Checks | Confirmation of DBS check
Right to Work and references will be needed. |
| Training | Training will be offered to enable to complete the mandatory training required:
• Safeguarding Adults • Introduction to first aid • Health and Safety

| What you would be doing… | You will be supporting with booking and attending hospital appointments, understand and relaying on going treatment plans in simple terms.
Support with: · Collecting medication from the pharmacy every 4 weekly. · Booking transport to hospital appointments · Support to the appointments and understanding treatment plans. · Support with appointments every 6 weeks for blood tests.
